i think what happens here is w zooms in, throws her knife at tifa, who rolled a nine and therefore gets it right in the shoulder, very close to important internal organs. and then vincent shoots, from his horrible position with his three - and the bullet misses her but is close enough to make her aim horrendous. she almost eats shit on the spikes as well.
